A 16 year old, happy young man is looking for a provider to assist him at home. He is on an IO Waiver and will need about 15-20 hours per week. He loves to go for walks and interact with others, even with limited language. Times and days will be worked out with the family. The individual lives in the Aurora area.
Family looking for several providers to work with 3 teenage boys, shifts and times will vary. Their needs vary for each individual. Family lives in the Chagrin Falls area and the family and other providers will assist in training. All 3 individuals are on IO Waiver
Looking for a provider (independent or agency) who is fairly fluent is American Sign Language to provide HPC/ATN services through the Level One waiver. This 19-year-old young man is looking to increase his community participation with the assistance of a provider who can help him communicate more effectively with hearing peers.
Chardon family looking for a LPN (or RN) for waiver Nursing services or State Plan nursing services in their home for 22-year-old son. Ideally, looking for hours from 3:15pm – 10pm Tuesdays, Wednesdays and Thursdays and 10 hours every other Saturday. However, family would like to interview anyone who is qualified and interested to cover as many of the hours as possible. Some services needed include checking pulse, oxygen level, oral suctioning, G/J tube feeding and medication administration, positioning and transfers in wheelchair as needed, assistance with all ADL’s. Provider would also assist with community and recreational activities.
Looking for a provider to work with a 19 year old young man in Chardon with establishing daily routines and schedules to be better prepared for daily activities and regular hygiene tasks. Times and days will be worked out with the family. He is on a Level 1 Waiver. He loves video games, music, and is talkative. Also, looking to become more involved in the community.
RFP for an individual who is looking for in home services for the following days and times: Monday through Friday, 2:30 p.m. to 6:30/7:30 p.m. with occasional weekend hours. This individual is still in school and would like after school hours. She enjoys going on outings and activities such as to the YMCA, library, Metzenbaum and more. She requires a high energy staff who can maintain her attention while caring for her. She requires physical assistance with some activities (toileting and hygiene) and a lot of redirection and verbal prompting. She uses a talker and needs to be encouraged to do so.
