I am a 20-year-old young man who likes to be around people. I am looking for a provider to assist me in my home after I get home from school. I am in a manual wheelchair and I don’t have control over my chair. I would be totally dependent on my provider to assist me with moving from one place to another place. I weigh about 80 lbs. and need help with moving from my wheelchair to the bed or living room floor. I live with my family in the Novelty area. I am on an IO Waiver and basically will need help on Monday and Tuesday, between 2:30-6 p.m., until one of my family members returns home.
I am looking for a provider for in-home services for the following days and times: Monday through Friday, 2:30 p.m. to 6:30/7:30 p.m. with occasional weekend hours. I am still in school and would like after school hours. I enjoy going on outings and activities such as to the YMCA, library, Metzenbaum Center and more. I require high-energy staff who can maintain my attention while caring for me. I also require physical assistance with some activities (toileting and hygiene) and a lot of redirection and verbal prompting. I use a talker and I need to be encouraged to do so.
