Becoming a Provider
Provider needed for a 27 year old male that lives in the Novelty area. He is seeking respite care in his home from July 2 through July 9 2017. He needs assistance with all daily living tasks and meals. He would be attending his day program during those dates except when closed for the weekend and the holiday. He would prefer someone stay in his home with him. The family will train the provider on all the details. If interested please respond ASAP.
Provider needed for a 34 year old male that lives in Chardon. Individual has Cerebral Palsy and Epilepsy. Individual is very social and loves interacting with those around him. He loves watching movies, playing board games or playing with his cars. He also loves listening to music. He will need assistance with medication administration, feeding, as well as all daily living skills. Provider will also be responsible for monitoring seizure activity. Provider is needed Monday-Thursday 3:00pm- 9:00pm, Friday 3:00pm-midnight. There is also a flexible 12 hour shift on the weekends (shift hours will be worked out with family) Family is seeking a provider(s) for partial coverage or full coverage of the above listed hours. 26-year-old female is seeking a provider to assist her approximately 10 hours per week. Needs assistance in completing recreational and leisure activities on Saturdays and Sundays from about 9 a.m. to 2 p.m. Individual enjoys doing different activities and needs assistance to plan and get to them.
